Overview

It is important for us to collect information about your child.

We need to collect certain information to satisfy our statutory obligations and to ensure speedy and accurate communication, in the case of an emergency.

This form must be completed by the person with Legal Parental Responsibility (PR) before your child can participate in community clubs.

The definition of legal PR (Children (NI) Order, 1995) is:

Natural mother always has PR, unless the child has been legally adopted.

Natural father has PR if: married to mother at time of birth, or is registered on the child's birth certificate, or gains PR through the courts. Or, by subsequently marrying the mother, through an agreement witnessed by a solicitor, through a Parental Responsibility Order, by a court (post 2002), if jointly registered on the child's birth certificate.
