Dementia Friendly Big Game Responses

We have set up this survey to allow you to enter the responses of the consultations you have undertaken with those living with dementia and their family/carers. 

We hope that this survey makes it easier to put forward the feedback received and once all feedback has been uploaded, a report outlining the findings will be shared. 

This information will help form the foundation of a regional action plan focused on supporting those living with dementia and their family/carers.
